－The removal of thread and the impression taking of maxilla and mandibula
before
prosthesis of artificial teeth－ No.293-Apr. 28, 202１ |
Today, the following two operation was performed.
(1)The removal of thread at the part stitched at the time of the last reinstallation of
healing-cap to the implant body embedded in the right mandibula
(2)The impression taking of maxilla and mandibula before prosthesis of
artificial teeth
scheduled for.
In the removal of thread, the threads protruding through the gums is just
cut off with
dental scissors.
The impression taking in the case of (2) means taking a dental impression
in the
patient's language.

There have been the cases where the impressions were taken, but this time
the meaning is a little different.
The method of fixing my artificial tooth is the screw-retention method.
So that means not the type to fix the artificial tooth with glue but with
screw.
In this case, the artificial tooth is connected to an embedded implant
body through
a connecting part called as the abutment

This time, the impressions were taken with the temporary abutment fixed
to the
implant body.
It is presumed that this is to fit the artificial teeth to the dentition
without any discomfort.
In the case of the screw-retention method, the artificial tooth can be repaired when it is
broken, but there is the demerit that the artificial tooth loosens as it
is used.
On the other hand, the adhesion method has the characteristic that the
artificial tooth
is less likely to come off because it is almost perfectly fixed to the
implant body.
But there is the demerit that the artificial tooth can't be detached and
so is difficult to
repair in the case of trouble.
The screw-retention method is adopted at M dental clinic.
My doctor said to me that he will temporarily attach the completed artificial
teeth and
fine-tune them next time.

「インプラント治療記(8)-抜糸と人工歯補綴前の上下顎の印象採取-」
Today's schedule are as follows.
(1)The sture removal after surgery to form the gingiva around implant bodies
embedded
in the left maxilla
(2)The impression taking to attach the crown to the right mandibula
But the impression taking was hastily cancelled.
It's because I told my doctor that one of the healing-caps attached to
the right
mandibula came off again 3 days ago.
For this reason, he switched to the operation for attaching the healing-cap
again after
the sture removal.
Since the gingiva has been already covered on the trace of the cap that
came off, it was
removed and then the cap was attached again.

In this surgery, I was injected with anesthesia in more than 10 places
around
the gums as usual.
So, I felt pricks of pain at first.
As expected, the anesthesia may have worked in the latter half of the surgery.
I finally became insensitive to pain.
Even when the gums covered on the trace of the cap were removed, I didn't
feel any
pain.
The effect of anesthesia is great, isn't it?

I left the clinic after receiving the lalixin tablets that is pyostatic
antibiotic, pain-relief
roxonin tablets and a bottle of mouthwash.
Somehow, I geel that I have many oppotunities to take loxonin tablets these
days.

「インプラント治療記(7)-抜糸とヒーリングキャップの再取付け-」
My surgical guide has already been completed based on the image inforrmation
obtained from the previous CT-scan.
This time, the surgery embedding titanium implant bodies is performed using
this guide.
Here is the thing.
Last night, one of the healing cap screwed to the implant bodies (or fixture)
being
embedded in the lower right jaw came off.
So, before the surgery, I had it re-screwed.
Healing cap is a tentative cap screwed to the implant body until the gingiva
is formed
around the implant body after its embedding.

Subsequently, the embedding surgery of implant bodies in the missing part
of the
maxillary molars was performed.
Just like the case of the mandibular molars (Daiary-2.1016), it was performed
in the
following order.

The surgery was completed in about an hour.
After I heard a story about some precautions
after the surgery from a female dental assistant, I received an antibiotic
azithromycin tablet (3 day's worth),
a pain-relieving loxonin tablet (2 doses),
and a mouthwash (10 day's worth).

When I was about to go home, I recieved a bank account transfer request
paper for
self-pay medical-care expenses from the reception staff same as last time.
According to my doctor, he plans to do implant prosthetic surgery (secondary
implant
surgery) next time because the condition after the embedding surgery of
implant bodies
to the lower jaw is good.
So, I'm sure that it is the expenses for making my artificial crowns.
Somehow, my current assets are just decreasing.
It's unavoidable, isn't it?

「インプラント治療記(6)-上顎にインプラント体の埋入-」
The implant bodies are already embedded in the missing traces of the lower
right
2nd and 3rd molars, and are waiting installation of the artificial crown
(Secondary
implant prosthetic surgery).
This time, I visited the dental clinic to take a 3D image of the upper
left jaw by
CT-scan.
The dental CT scanner is a 3DX Multi Image Micro CT manufactured by Morita
Co.,Ltd.

The position, direction, angle, etc. embedding implant bodies can be confirmed
by simulating 3D image taken
by the dental CT scanner.
And then a surgical guide with holes of a certain size
at the position where the drill is inserted is fabricated.
In an actual surgery, this surgical guide is attached
to my teeth.
The drill advances along the hole dug in the guide.
Therefore, the holl with the prearranged position, direction, depth is
drilled safely with pinpoint accuracy
on my jawbone and then the implant bodies are
embedded.

According to my doctor, the gums near the maxillary molars after suture
removal are
in good condition.
So, next time (on April 9), the embedding surgery of implant bodies in
the upper jaw
will be performed using the surgical guide fabricated.
Just before I got home, I recieved something I didn't like.
It was a bank account trnsfer request paper for self-pay medical-care expenses.
I want to make me pay for the treatment till I'm ready.

「インプラント治療記(5)-上顎のCT-scan-」
In the intermittent rain, I went to M clinic connecting with buses and
JR train as usual.
I arrived at 14:50, 10 minutes before the appointment time.
I stood by to contact my doctor in the lobby after the standard coronavirus
check, that
is to say, measurement of body temperature at the reception desk and gargling
with
dense Isodine three times at the hand-washing place.
Last time I received the embedding surgery (primary surgery) of two implant
bodys
to the portion missing from molar teeth of the lower jaw, so today had
a subsequent checkup and my stitches removed.
My doctor said it has been normal since the surgery.
They were competed in about 10 minutes.

After that, I asked my doctor for temporary treatment for my central incisor
that has
always been wobbly and about to come off.
Though I plan to get implant treatment for this tooth as well, I tentatively
got temporary
treatment.
According to my doctor, the tooth root get thin and so broken.
As a stopgap measures, I had this tooth bonded to the adjacent teeth.

Well, I was very surprised at the power of dental superglue.
After glueing, he sharpened the teeth with a dental handpiece many times
to
fine-adjust my dental bite.
But despite the strong vibration when sharpening teeth, it wasn't move
at all.

to be able to
have a meal for the time being.
Anyway, I want to have a safe and secure diet by radical treatment with
implant as
soon as possible.
My doctor said to me that he plans to do CT-scan on my upper jaw next
time for
having primary implant surgery to the portion where the molar tooth, which is the false
tooth, was removed.
Since I'm going to enjoy golf with members of Ikoikai Society reunion at
Kasumidai
CC in Ibaraki Prefecture, I made the next reservation at the end of March.
